An employee is leaving the University. How can I have their BSD account(s) disabled?

Send an email from the person's supervisor, PI or EA to help@bsd.uchicago.edu with the information listed below.

Note: If the request is urgent, you may call us at 2-2056, but we will still require a written request from the person's supervisor, EA or PI.

1. Submitter's name
Submitter's email address
Submitter's phone number

2. Terminate account for: [name]
Termination date:
Supervisor's name:
Supervisor's phone number

3. Will this employee need access to their email account beyond the termination date? If so, for how long? (Maximum 90 days)

4. Will the employee's email need to be forwarded to another BSD or UCHospitals account? If so, please specify the account.

5. Do you need an Out of Office autoreply to be set on the meployee's email account? If yes, please provide the text of the message you wish others to receive when sending to this address, and the desired duration. (Maximum 90 days)

6. Will the supervisor or someone else will need access to terminated employee's personal share? (Maximum 30 days)
